New Patient Paperwork

What to Expect

New patient visits take 1 hour

• Be prepared to be in the office for at least 90 minutes
• Do not bring children other than the patient to the visit
• If you are more than 20 minutes late for the appointment, you may be asked to reschedule.

Preparation will use your time most wisely

• Please complete all of the new patient paperwork
• Please have records sent from all the patient’s other physicians, any hospitals the patient has been in (including the birth hospital)
• Most importantly, if the patient has had any genetic testing at any time, please make sure we have those test results.

Concerns about privacy and genetics

• If there is any information that you wish to discuss with Dr. Scheuerle privately, please let the office staff know before the visit.
• Testing is not mandatory
• Please discuss your concerns about privacy and insurance with the doctor.
